This document is essential in every circumstance where an employee's employment is being terminated from Part-Time employment.

Termination of an employee should always be in writing and it should record the reasons for termination. This can be of vital importance if there is a personal grievance, constructive or unjustified dismissal claim brought by a former employee.

A copy of this form should be kept in the employee's file as well as in your general employment records.
